You could save money with a stand-alone Community CCRxSM Basic or Choice (PDP) Medicare Part D Prescription Drug Plans. Remember, our Part D formularies (list of covered drugs) cover a wide range of drugs specifically designed for people with Medicare, and not covered by Medicare Part A or Part B (Original Medicare). For 2012, we offer two stand-alone Prescription Drug Plans, Community CCRx Basic (PDP), and Community CCRx Choice (PDP). Plan features include:

*The $0 copay applies to up to a 30-day supply of a Tier 1 drug for Community CCRx Choice (PDP) during the Initial Coverage Stage (ICS), which for this plan begins on the first day of coverage. The $2 copay applies to up to a 30-day supply of a Tier 1 drug for Community CCRx Basic (PDP) during the ICS, which for this plan begins once the deductible is met.

Community CCRx has a network of pharmacies. To receive plan prescription drug benefits, you must use a network pharmacy, except in non-routine circumstances, and quantity limitations and restriction may apply.
You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you qualify for getting Extra Help, call:
